Terms and Conditions
By accepting a booking for an assessment, you, the client are agreeing to the following terms and conditions.
An assessment will include a full battery of relevant tests and a full diagnostic report. The arrangement and fee is for one such assessment, no part of the assessment may be separated off as an individual item.
Any additional screening required will be discussed with you directly and the cost agreed upon before the administration of tests/screening materials. Further screening is not automatically included in a full diagnostic assessment for dyslexia.
Clients can expect a report within approximately 2 weeks of the assessment date (10 working days).
The full assessment fee is due before a booking is confirmed. Once payment has been received, a date will be confirmed with the client. The client will receive booking information, relevant documentation and links to the assessment/appointment within 24 hours.
Cooling off period: The client will receive a full refund should they change their mind within 7 days of booking and paying for an assessment. Please note that if the assessment date is scheduled within the 7-day cooling-off period, normal cancellation fees apply. This normally happens if an emergency request has been made to assess a client at short notice.
Cancellation Policy: Whilst we make every effort to work with you in the event of a cancellation, cancellations up to 15 days before the assessment date will incur a £100 admin fee.
A refund will be made to the client, less a cancellation fee (£150) if the booking is cancelled within 14 days before the assessment date.
A refund will be made to the client, less a cancellation fee (£250) if the booking is cancelled within 7 days before the assessment date.
Cancellations within 48 hours prior to the assessment will result in the client forfeiting the full cost of the assessment.
If your assessor has to cancel an assessment appointment you will be offered an alternative date if possible. If this is not possible you will receive a full refund.
You have a right to complain to PATOSS should you feel the need to. Please use my membership number as a reference. This is displayed on the diagnostic assessment report.
Your data will be kept electronically. Only the data which is needed to complete the assessment will be kept and the data needed to contact you about the assessment. The data needed to complete the assessment will be kept electronically for no less than 6 months. Please note that it is the client's responsibility to save a copy of their diagnostic report.
Your electronic data is kept securely and will not be used for any purpose other than to produce the diagnostic report. It will not be passed onto third parties (such as your college, university, employer or caregiver) without your permission. All physical test papers are destroyed after the diagnostic report has been written and released to the client.

If your assessment is financed by your college/university and you fail to comply with the terms and conditions of the assessment, you may be liable for the full cost of the assessment.
